There are a variety of ways to refer for Services at Pathways Health Centre for Children:
- ask your physician for a referral
- ask your current health professional if you are involved with other services at Pathways Health Centre for Children
- complete a Request for Service Form and mail or fax it to us (see below)
- complete an online Request for Service Form
